We all make mistakes in our lives, especially in tech, where any new piece of code can introduce a [[Software Bug]] easily. Sometimes the error is found and fixed before a client is able to experience it, but as we know, errors are common in the world. If the company you work for has a culture of blaming specific people for a problem instead of analyzing why did this problem happen and what we can do better, it might be time to [[Job hunt to your hearts content]]. Nobody needs to be paranoid over doing something that will happen no matter what. All we can do is work together to understand why, fix it, and create counter-measures, that way it doesn't happen again.
